Hi, your MID-cap is quite large, while your REFP-cap is rather small... REFP-resistor is smaller than in the datasheet, too. Dunno if that matters, but... The MID cap is the same as the one on my devboard, as is the REFP cap. And since the devboard works fine, I don't see how they would be a proble...

It`s clear why you put those diodes before the input of U1. Try that cap before pins1/2. We do not know how the DC voltage of those pins behave during power-up; One of the diodes might get "DC-conductive" from the chip`s wandering input-pin DC-voltage... That`s different from the case how ...

Are D14 & D15 orientated correctly on the PCB?
Try a capacitor (e.g. 1uF) between the node R9/D14/D15/C13 and the pins1/2 of the FV-1.
Or you could first try to lift/disconnect one leg of each of those two diodes,
and check if the circuit still misbehaves...

Your R2 ties the input to ground - not good. R1/C3 is 10k/2n2 (better: 1k/1n) You don`t have a simple Low-Pass-Filter at the output (1k/2n2). The signal path is very easy: guitar-->buffer-->fv-1 board-->buffer-->amp I am driving the ADC input with guitar level which is too cold but i expected little less output noise.
